Entry into enclosed space and should be completed by the Master or responsible officer and by the person entering the space or authorised team leader.
GENERAL
1. Location/name of enclosed space
2. Reason for entry
3. Validity from (date and time) to (date and time)PRE-ENTRY PREPARATION
1. Thorough ventilation
2. Mechanical/electrical isolation
3. Space cleaned if necessary
4. Gas analyser calibrated, test atmosphere and record readings
* Oxygen 21%, Hydrocarbon <1 % LFL, Toxic gases Specific gas and PEL ppm
5. Arrange atmosphere check at regular intervals
6. Arrange continuous ventilation
7. Adequate access and illumination
8. Rescue equipment standby
9. Responsible person standby at entrance
10. Inform OOW/EOW
11. Communication check and emergency signals discussed
12. Emergency and evacuation procedure
13. All equipment inspected, good condition
14. Proper PPEPRE-ENTRY CHECKS
1. Received permission
2. Pre-entry prep completed and signed
3. Agreed communication procedure
4. Agreed reporting interval
5. Agreed emergency and evacuation procedureBREATHING APPARATUS AND OTHER EQUIPMENT
1. All familiar with breathing apparatus
2. Breathing apparatus tested
* Air pressure gauge and reading recorded, Low pressure audible alarm, Face mask under positive pressure and not leaking
3. Test communication and agreed emergency signals
4. All personnel provided with rescue harnesses
5. Signatures + Date/time of Master/nominated person, Responsible person supervising entry, Person entering space or authorised team leaderPERSONNEL ENTRY
